Teacher Feedback Request (from parent)

Dear Teacher,

I recently learned about a feedback system at www.5to1leadership.com. I was encouraged by it’s focus on the character and leadership qualities of people. I was hoping you could help me provide support and encouragement for {child’s name} by providing 5 leadership qualities you see in {him/her} and why, along with 1 quality {he/she} could improve on and how?

Here is an example of 5:1 feedback from a teacher:

1: hard worker: he always does his best

2: humble: he likes to win but respects others

3: empathy: he has a true concern for others

4: trustworthy: he keeps his word

5: good friend: he values close relationships with friends and family

1. confidence: more comfortable in expressing himself
